All questions must be completed. Please read carefully the ‘Terms of Conditions of Trade’ and sign the agreement at the bottom of the page.

It is the customer’s legal obligation to notify Slipstream Creative Limited (The Company / Seller) of any changes to the information given on this application.

Payment terms are: Payment by 20th of the month following date of invoice. If printing is required a 50% deposit is required prior to sending the job to print. A quotation remains valid for 30 days from the date of quotation after which a revised quote may be necessary. Any variation to the scope of changes outlined on the quotation form may result in a variation to the quoted price.

For a quotation or a scope variation, by signing below I acknowledge that I am accepting the conditions outlined in the quotation or scope variation. For a work approval I sign as acknowledgement that I authorise the works as and when specified to proceed.

By submitting this form I certify that the above information is true and correct. I have read and understood the TERMS AND CONDITIONS OF TRADE of Slipstream Creative Limited which form part of, and are intended to be read in conjunction with this Account Information Form and agree to be bound by these conditions. I authorise the use of my personal information as detailed in the Privacy Act clause therein. I agree that if I am a director/ shareholder (owning at least 15% of the shares) of the Buyer I shall be personally liable for the performance of the Buyer’s obligations under this contract.
